Mindful Behavior participates in a variety of health insurance products and plans. If Mindful Behavior is listed in your insurance company's network, you’ll be able to receive care at our facilities with your “in-network” benefits. That means your insurance company has agreed to accept certain negotiated rates, which can lower your out-of-pockets costs.

Insurance Plans

Our list of accepted insurance providers is subject to change at any time. You should contact your insurance company to confirm Mindful Behavior participate in their network before scheduling your appointment. Your insurance company's customer service representative can also help you verify your benefits, out-of-pocket costs and coverage.

If your insurance network does not include Mindful Behavior, but has out-of-network benefits, you may have a higher out-of-pocket responsibility than if you received services from an in-network provider.

  • Mindful Behavior offers the sliding scale for clients who dont have insurance. Please call the office for more details.

  • The good faith estimate shows the list of expected charges for items or services from your provider or facility. Because the good faith estimate is based on information known at the time your provider or facility creates the estimate, it won’t include any unknown or unexpected costs that may be added during your treatment. Please review our fees page to see all our fees.

  • Your insurance card should have a "Members Services" or "Customer Service" number, typically on the back. When speaking with your insurance, be prepared to share your ID number, name, date of birth, and social security number.

Medicaid

Mindful Behavior takes Medicaid, please be mindful of your medicaid coverage and insurance it is active before scheduling an appointment. Reminder that medicaid covers all sessions if we are in network with your medicaid plan, there will NEVER be any hidden fees with medicaid.

Hourly standard Rates

$250 for initial intake/evaluation sessions

$175 per individual, couples, family  sessions

$60 for 45 minutes individual session (self-pay)

$25 for 15 minutes of over time pass session time (self-pay)

$30 for 30 minute sessions (self-pay)

Reduced Rates

$150 for initial intake/evaluation 60 minutes- 53min in session

$100 for individual therapy 60 minutes- 53min in session

$150 for couples and family 60 minutes- 53min in session

$75 for 45 minutes individual session (self-pay)

$60 for consultation or 30 minute sessions (self-pay)

$35 for 15 minutes of over time passed session time (self-pay)

Other fees charged

$100 for clinical consultation/supervision

$150 per hour for school or event clinical presentations

$150 for filling out forms for example; FMLA, Doctor’s Notes, School Notes (Not included in session fee)

$50 if clinician has to drive to home/court/other agency

$800 for immigration evaluations

$75 for anger management and parenting classes (8 week class)

$500 for Firearm Evaluation (This is not a Forensic Psychological Evaluation) Deposit of $100 before booking eval.

$100 per hour for Independent Clinical Supervision

$250 per hour for Clinical Consultation

You are responsible for all charges. Payment is due at the time of service, no exceptions.

Be aware that it may take a few week to schedule an appointment if there are no current appointments available, however, our appointments are usually given within 48hrs

MB utilizes federal income guidelines to determine if a client qualifies for a sliding fee in certain circumstances

Self-Pay clients are required to leave a $50 deposit to secure their intake appointment

Starting September 1st, 2023, or no show/late-cancellation fee will be $100

Court rates: (No insurance will be accepted for court appearances)

$450 per hour for court appearance

$250 per hour for 215 Evaluation 

$3,500 for 604b Evaluation

$75 per hour if clinician has to drive to home/court/other agency
