Mindful Behavior participates in a variety of health insurance products and plans. If Mindful Behavior is listed in your insurance company's network, you’ll be able to receive care at our facilities with your “in-network” benefits. That means your insurance company has agreed to accept certain negotiated rates, which can lower your out-of-pockets costs.
Insurance Plans
Our list of accepted insurance providers is subject to change at any time. You should contact your insurance company to confirm Mindful Behavior participate in their network before scheduling your appointment. Your insurance company's customer service representative can also help you verify your benefits, out-of-pocket costs and coverage.
If your insurance network does not include Mindful Behavior, but has out-of-network benefits, you may have a higher out-of-pocket responsibility than if you received services from an in-network provider.

The good faith estimate shows the list of expected charges for items or services from your provider or facility. Because the good faith estimate is based on information known at the time your provider or facility creates the estimate, it won’t include any unknown or unexpected costs that may be added during your treatment. Your insurance card should have a "Members Services" or "Customer Service" number, typically on the back. When speaking with your insurance, be prepared to share your ID number, name, date of birth, and social security number.
Medicaid
Mindful Behavior takes Medicaid, please be mindful of your medicaid coverage and insurance it is active before scheduling an appointment. Reminder that medicaid covers all sessions if we are in network with your medicaid plan, there will NEVER be any hidden fees with medicaid.
